Pursuant to the State of New York Freedom of Information Law, Public Officers Law §§ 84, et seq., I hereby request copies of any and all inmate records of _____________ (Department Identification Number ______________, including, but not limited to, his (a) personal history, (b) correctional supervision history record, (c) guidance and counseling files, (d) disciplinary office file, (e) inmate central office file, (f) service unit file and (g) rap sheet. Mr. _______________ is currently incarcerated at the Great Meadows Correctional Facility.

I enclose a release signed by Mr. __________, authorizing the release of all such records to me.

We will, of course, pay for all costs of photocopying these documents.
